Route 1: Wegovy on the NHS for weight management
For weight management, Wegovy is available on the NHS only within a specialist weight management service, not through a standard GP appointment. NICE guidance (TA875) recommends it for people with at least one weight-related condition and a BMI of at least 35, or 30 to 34.9 in some cases, and it "must be used within a specialist weight management service providing multidisciplinary management of overweight or obesity".1

Route 2: Wegovy on the NHS to reduce heart attack and stroke risk
From 2026, there is a new NHS route that is not about weight loss alone. NICE now recommends semaglutide to help prevent heart attacks and strokes in adults who "have previously had a heart attack, stroke, or be living with peripheral arterial disease", who "have a body mass index (BMI) of 27 kg/m2 or above", and who are "willing to make changes to their diet and increase physical activity".3
NHS England estimates that around 1.2 million people with cardiovascular disease could be offered the treatment, delivered as a weekly injection alongside a healthy diet, more activity and other usual medicines such as statins.4 In the SELECT trial, published in the New England Journal of Medicine, semaglutide reduced the risk of serious cardiovascular events by around 20% compared with a placebo in people with cardiovascular disease who were overweight or obese.45

Why is Wegovy so hard to get on the NHS?
Wegovy is hard to get on the NHS because access is deliberately narrow and demand is high. For weight management, prescribing is limited to specialist services with finite capacity and strict BMI and condition criteria, and treatment is capped at two years.1 Many people either do not meet the thresholds or face long waits for a specialist referral. The new cardiovascular route is broader, but it targets people with existing heart or circulatory disease, so it does not help most people who want treatment for weight alone.
What if you are not eligible for Wegovy on the NHS?
If you do not meet the NHS criteria, Wegovy can be accessed privately.
